CU Online Electronic Banking Service Agreement

Internet Banking is available to most credit union account holders. You must be a primary or joint owner on an account to qualify. During the enrollment process you will create an Internet Banking Username and Password. This will be used to manage all of your consumer accounts at the credit union.

By enrolling in Internet Banking, you certify that you: (a) or at least one account owner is 18 years of age or older; (b) are a United States resident; (c) are legally capable of entering into contracts; and (d) have and will register Payment Account(s) in your name on which you are an account holder or Authorized user. You agree that some or all of the registration information you provide may be subject to verification by FORUM Credit Union at any time. At the present time, you may use Internet Banking to:

  • Apply for a loan
  • Transfer funds between your accounts and to other unrelated FORUM accounts
  • Obtain balance information for your savings, checking, Loan and Certificate accounts
  • Receive email alerts regarding a variety of account activity
  • Make loan payments from your savings and checking accounts
  • Make bill payments to creditors from your account
  • Place a stop payment on checks
  • Reorder checks
  • Correspond with FORUM Credit Union employees via secure E-mail
  • If enabled, view periodic statements via online e-Statements
  • Download account information into financial management software programs for reconciliation
  • View online check images
  • If enabled, make deposits from your computer using your own scanning device and software we provide

Federal E-SIGN Act Disclosure and Consent

Please read this Federal Electronic Signatures in Global and National Commerce Act ("E-Sign") Disclosure and Consent carefully and keep a copy for your records. You can record your consent to the E-SIGN disclosure by clicking the Agree button below. By clicking the Agree button below, you consent to the electronic delivery of periodic statements, disclosures, agreements, change notices, terms and conditions and any other documents. You also agree that we do not need to provide you with additional paper (non-electronic) copies of periodic statements, disclosures, agreements, change notices, terms and conditions and any other documents, unless specifically requested. Once you consent to periodic statements, you may request paper copies, which may result in a fee.

Electronic Delivery of Periodic Statements, Disclosures and Notices

In order to use the Online Account Opening Product and to access, receive and retain periodic statements, disclosures, agreements, change notices, terms and conditions and any other documents you must provide, at your own expense, an Internet connected device that is compatible with this financial institution's Online Account Opening product deployed at the time of access. Your device must meet the minimum requirements outlined below. You also confirm that your device will meet these specifications and requirements and will permit you to access and retain periodic statements, disclosures, agreements, change notices, terms and conditions and any other documents electronically each time you access and use the Online Account Opening product. Please click on the HTML links to review the disclosures prior to continuing with your application. In order to retain, you may print the documents by selecting "Download". Select your printer and click on OK or select Save to retain a copy on your PC. If you are working on a public computer, you should not engage in online account opening. If you do not want to receive periodic statements, disclosures, agreements, change notices, terms and conditions and any other documents electronically, you should exit this area of our website. If you do not consent to receiving these documents electronically, you will not be able to open the deposit account via our website.

System Requirements to Access Information

To receive an electronic copies of periodic statements, disclosures, agreements, change notices, terms and conditions and any other documents you must have the following equipment and software:

  • An up to date Internet browser that we support (see Browser Recommendations)
  • Sufficient electronic storage capacity on your computer's hard drive or other data storage unit
  • An e-mail account with an Internet service provider and e-mail software in order to participate in our electronic Communications programs
  • A personal computer, operating system and telecommunications connections to the Internet capable of receiving, accessing, displaying, and either printing or storing Communications received from us in electronic form via a plain text-formatted e-mail or by access to our web site using one of the browsers specified above
  • Software that enables you to view files in the Portable Document Format ("PDF)

Withdrawal of Electronic Acceptance of Periodic Statements, Disclosures and Notices

You may withdraw your consent to receive periodic statements in electronic form and instead receive paper statements (for a fee) or disclosures, agreements, change notices, terms and conditions and any other documents for any of your Accounts by contacting us via email using the "Contact Us" tab on our website www.forumcu.com or by telephone at 317-558-6000/800-382-5414, option 3 or by postal mail at P.O. Box 50738, INDIANAPOLIS, IN 46250. We may treat your provision of an invalid email address or the subsequent malfunction of a previously valid address or cancellation of participation in our online banking product as a withdrawal of your consent to receive electronic statements. We may impose a fee to process the withdrawal of your consent to receive electronic statements. Any withdrawal of your consent to receive electronic statements will be effective only after we have a reasonable period of time to process your withdrawal.

How to Update Your Records

It is your responsibility to provide us with true, accurate and complete e-mail address, contact, and other information related to this Disclosure and your Account(s), and to maintain and update promptly any changes in this information. You can update such information (such as your e-mail address) at our website www.forumcu.com or by telephone at 317-558-6000/800-382-5414, option 3 or by postal mail at P.O. Box 50738, INDIANAPOLIS, IN 46250.

I/We confirm that I/We have read and agree to the CU Online Electronic Banking Service Agreement and Federal E-SIGN Act Disclosure above and any amendments to this document from time to time that collectively govern my/our membership and accounts.
